Guy’s and St Thomas’ Charity – transforming general practice provision​

NHS Lambeth Clinical Commissioning Group (CCG) applied to Guy’s and St Thomas’ charity in partnership with NHS Southwark Clinical Commissioning Group and our respective Local Medical Committees (LMCs), to support the development of primary care over three phases.

At the core of this programme was the development of ‘Emerging Leaders’ for Lambeth who were tasked with engaging  practices within their respective localities to help create a shared vision for primary care and new ways of working that enable this vision to be achieved. 

This is a complex change programme that requires significant resource. NHS Lambeth CCG submitted a bid for the second phase in September 2014, which has built on the outputs from phase one and provides further investment to agree a shared vision across Lambeth and Southwark CCGs. 

A bid for the third phase will be submitted over the next few months and will identify the investment required to enable general practice to implement new models of primary care that deliver on the overall ambitions of this programme of work.

Our overall objective is to build leadership and capability to transform general practice across the boroughs of Lambeth and Southwark by developing integrated models of primary care that are bottom-up and clinically driven.

By investing in leadership and development, member practices can continue to drive innovation and collaborate in new ways of working that take account of the local context and deliver more integrated models of care.
