The Community Incentive Scheme (CIS)
Lambeth Clinical Commissioning Group (CCG) is developing a Community Incentive Scheme (CIS) to support GP practices to work with people who have serious mental illness (SMI) and been discharged from South London and Maudsley (SLaM) services in Lambeth.
As part of the Living Well Network Hub, an element of the Primary Care Nursing Liaison will be funded. The Hub is a collaboration between SLaM, Lambeth Council, Clapham SPMS (specialist provider of medical services) who manage nurses with the Primary Care Support Service, Thamesreach who provide social support and Missing Link, a peer support group who manage peer supporters.
Practice responsibilities will include prescribing, monitoring and administration of medication, regular and comprehensive reviews, proactive follow-up, comprehensive risk assessment, and collaborative care planning.
Participating practices will have access to:
- A named, qualified mental health liaison nurse from the Hub team
- A named link worker from the local Community Mental Health Team
- Hub services including peer support, psychiatric assessment, social support
- A Locality Mental Health Clinical Lead
- A primary care based project facilitator
The CIS will include a range of patient needs from medication, monitoring and maintenance to more intensive support.
