What are the Local Care Networks?
Lambeth health and care providers (including voluntary sector and patient/ citizen representatives) are coming together in Local Care Networks (LCNs) to work collaboratively to address common challenges. Through these networks, providers can look at the range of services that they provide for our local population and see how they can work together to better join up care and improve health and wellbeing outcomes for local people.
Over time it is expected that LCNs will hold contracts for particular population cohorts. At present they act as multi-provider boards - working across organisational boundaries, and coordinating efforts to re-design services for people with complex needs.
Who are we looking for?
We are seeking three exceptional individuals to chair a LCN in each of the Lambeth localities – North, South East and South West. We need individuals who are committed to prevention, empowering patients and engaging communities in developing new models of care that deliver better patient/ carer experience, improved health and care outcomes and reduced reliance on secondary and primary care services.
The post holders will require strong leadership skills and ability to drive complex agendas forward, while bringing people with them. The successful applicants will have a comprehensive understanding of the local health and care system. Read the job description here.
During 2017/18 Lambeth LCNs will move from being a 'coalition of the willing' to more formalised partnerships. This will require the LCN Chairs to have robust understanding of the local and national political agenda as well as strong negotiation and influencing skills and understanding of the importance of value based care.
