Lambeth pilot for GP Standard Operating Procedure

Guy's and St Thomas' Trust (GSTT) has recently launched a pilot to give Lambeth GPs direct telephone access to an acute medicine consultant to discuss whether a patient's admission to hospital can be avoided.

The GP SOP Line is 020 7188 2514, operating Monday to Friday from 11am to 1pm and 4pm to 6pm. This corresponds with the estimated peak activity from GP surgery times.

Inclusion Criteria

• Adult (16 years +) patients can be discussed via the GP Line.
There is an option for the GP practice to arrange the transfer of patients via a paramedic crew.

Exclusion Criteria

• Acutely unwell patients who require immediate hospital emergency treatment - these patients will still be required to attend the emergency department.

If the joint decision is made to bring the patient to the Acute Assessment Unit (AAU) at St Thomas’ Hospital, the GP practice will be emailed a patient leaflet with location details.

Clinical notes will be available to the GP via the Local Care Record. The notes can be emailed to a practice on request via the ward clerk and NHS.net.

Feedback on the pilot will be collated from GP practices for a review at the end of March.
