Our GP practice members

NHS Lambeth CCG is a membership organisation. We are made up of 41 GP practices across Lambeth. As members of the CCG they guide the organisation. They also make sure the CCG is getting the most from the money it is allocated from the Government.

Our CCG Constitution sets out the way we operate. As a member organisation we work through the Lambeth Collaborative Forum and Governing Body to ensure health services are commissioned effectively and meet the needs of local people. GP commissioning means we can focus on the quality and clinical effectiveness more than ever before because Lambeth clinicians know the NHS and how services work because they talk to patients every day.
We work in line with our agreed mission, vision and values. The CCG is made up of three localities, North Lambeth, South East Lambeth and South West Lambeth together comprising all of our 41 member GP practices.
Primary Care Networks
Primary care and community care are at the heart of the NHS Long term plan. The plan sets out an ambition for all GP practices to come together with neighbouring and associated practices in "Primary Care Networks" (PCN) to meet the needs of local populations.
PCNs are much more than groups of general practice. They are about groups of GP practices working together locally in partnership with community services, social care and other providers of health and care services around the needs of local patients.
Our Healthier South East London is very pleased to announce that we have now agreed the development of 35 PCNs across south east London and the contracts formalising these arrangements between local GP practices, will go live today (1 July 2019).
As part of a PCN, GPs will be able to work with multi-disciplinary teams, including pharmacists, physiotherapists, paramedics, physician associates and social prescribing support workers. This will help free-up family doctors and enable patients to access the care and support they need from a wider range of professionals in the community.
In the first year of the PCN contract, the focus will be on optimising current services across the network, such as the provision of extended opening hours for local practices and the development clinical pharmacists and social prescribing link workers.
It is expected that the capability and capacity of PCNs will build over the next five years of the contract to deliver fully integrated community-based care that meets the needs of our local populations.
