Pulross Intermediate Care Centre and the Lambeth Supported Discharge Team
The Pulross Intermediate Care Centre and the Lambeth Supported Discharge Team worked collaboratively to support a patient and his family during a difficult time in their lives. The patient was moving to long term care from London to the north of England. The team wholly embraced the Guy’s and St Thomas’ NHS Foundation Trust value of ‘putting patient’s first’. They supported the patient’s discharge home from hospital for two days before he moved into long term care.
An excerpt from an email sent by the patient’s son, demonstrates how much the care and support provided by the teams meant to the family and how it had such a positive impact on their lives. It also demonstrates the high standards of care and professionalism the teams are providing:
“My father was discharged from the Pulross Clinic. It was his wish to spend 48 hours at his house before leaving to go to a care home. The discharge co-ordinator at the Pulross, Rachel, organised through the Whittington Centre for four teams of two to come and assist dad whilst he was at his own house.
The team provided the most professional level of care to him. They were patient, pleasant, efficient and listened to the needs of my father. It was his wish to go into the garden to see what progress had been made during his long stay in hospital. This was achieved with good humour and considerable skill on their behalf. There was no indication that they were working to a rigid time constraint and they made dad feel completely at ease.
