Lambeth integrated adult programme brings together our planned and unplanned work into a single programme focussed on four key areas:
• Urgent Care
• Older People
• Long term conditions
• Elective Care

The integration focus for adult services is aligned with Southwark and Lambeth Integrated Care Programme (SLIC), working with Southwark Clinical Commissioning Group (CCG) on planning and engagement with Guy’s and St. Thomas’, King’s and St George’s acute trusts. The programme will support system sustainability work across South East London.

  • To improve integrated services to  provide better health and wellbeing outcomes for patients
  • High quality and cost effective health and care system
  • Delivery of financially sustainable health care system for Lambeth

Lambeth Clinical Commissioning Group (LCCG) is committed to ensuring there are systems and processes in place to promote the safeguarding and wellbeing of the people we serve. We monitor and develop practice in light of local and national guidelines and work in partnership with patients, their families, carers and other agencies to ensure safe environments for all patients.
